3.23.24 Calling Insurance to Verify 100% Coverage

I call my insurance company to ask about coverage for the surgery. The agent on the phone tells me what I saw online, citing co-insurance, deductibles, etc. I say it was my understanding that, as a preventative procedure, it should be covered at 100%, and how can I get it covered? The agent tells me my doctor simply needs to submit a preauthorization form stating this is a preventative procedure and she recommends this particular surgery for me. He says if for some reason that does not work, they also have a 2-level appeals process. He’s much nicer and more helpful than I anticipated.
